You can usually fix this by running your phone until it shuts down by itself, then plug it in a good long time (over night is best) then let it run down again until it shuts off and charge one more time over night. If that doesn't fix the issue I would make sure you have a good backup (encrypted iTunes backup is best) and do a restore. Mine often shows a higher reading when I am in an app than it does when I am at the home screen.
