I have one customer we are dealing with this. Only happens on his Apple devices (iPad, etc.) I can't reproduce it in the lab. Tried adjusting his MTU (PPPoE customer) and other stuff. No Joy. If you can, try reducing the MTU down to say 1300 Bytes and clamp the MSS to 1280 on a customer router to see if the problem goes away, if it does, you'll need to work out where the MTU issue is on your network and resolve it.
