We have been using VoX for several years.  They are a member.  Perfect dial

M2/VoipInnovations includes it as part of each line for $1 or less depending
on the tier you have.  My average line including e911, cnam, and directory
listing is just under $4.  I know some companies want a $1500 setup fee, or
an extra monthly fee, but they do not.

We have multiple termination providers like Gafachi, VoipJet, and a few
others for failover or calculated rate pricing depending on the terminating
DID.

We have an individual account with Gafachi for testing and such.  Pretty
slick and great if you're just starting out.  The price is a bit higher so
if you're doing heavy volume you'll want to switch.
