google for exactly this and you should find relevant information. Your best bet to get sound in safemode is with an usb sound device that uses windows native usb sound drivers. There is a registry hack floating around the net that enables sound in safemode and it works down to windows xp

Download below 7z file and extract it. There is 2 .reg files one for activating audio in safe mode and second is for deactivating. Just enter the activation file and click OK. Restart the windows to Safe mode. You can use NVDA on safe mode without problems. JAWS has video intercept issue,
