You need to take a printout of the application you submitted and other
documents you might have uploaded and take them to the CMO of the District
Hospital that is relevant to you. They will verify the application. If you have
a valid Disability certificate then you should get the UDID
Please raise a rti with application Id. I did and got approved in one day,
which was been waiting for 2 years.

Well their application would have been verified by the CMOs office. No hospital
