Hrm, Miasma could work as a more granular kind of system than cards. Maybe have Miasma go from 0 to 100, and minor punishments grant 0-25 Miasma, while more severe ones grant 25-50. You'd lose 10 Miasma each month, and depending on how much Miasma you've got, you'd be more or less "sick" with
