I would suggest to enable SQL-Logging in ar.cfg. Add the following line.
Debug-mode: 1
Restart ARServer and post the arsql.log

Had similar issue with a 7.5 install. The main issue was that the
installer does not like if you want to create a db user widt password
with a number as the first char.
Had to remove the database, aradmin user and the arole role and then
start overagain. And then just use the defualt values.

Have you checked if the entry for the Alerts form does exist in the arschema
table? (Select count(*) from arschema where name = 'Alert Events';)

Try this sql:
select count(rowid) from user_cache
/
The fact that user_x is missing is worrisome. This more than likely means
that the user form does not exist on the server.
Try using arcache to create a temporary admin user. See the ARS config
guide for instructions and examples.

This means that the H and T tables that are the underlying tables for the said
form have perhaps been created already.
To verify this first of all find out the schema ID for that form.
select schemaid from arscheama where name = 'Alert Events';
