To me, unlimited means unlimited. If you didn't mean it, why did you sell
it as such?
Fair use policies are bogus IMHO.

As long as the data downloaded/uploaded and the internet activity is not
illegal then unlimited would stand fair usage normally would cover you for
illegal activity.

Fair use is fair use. If they're costing you more than you're making off
them, it's unsustainable. I'd have a conversation with the customer
around that very fact, they're either oblivious to the problem, or
oblivious to the impact it's having on you.
Memory is somewhat vague now but I have
I agree Unlimited is Unlimited.
That being said, it certainly wouldn’t hurt to talk to them and explain the
situation.

Surely the service contract outlined the conditions that would breach fair
use?
If I found a nice price point 20/20 unlimited service to sync offsite
backups or something else data hungry, as a customer I'd class that as fair
use of the service.

A 20Mbps service is a 20Mbps service. If they are running it at the
limit all the time then perhaps have a discussion with them to check
that they haven't got a run away process doing that and if not then it's
an opportunity to upsell them into something with some extra headroom.

Fully agree here too, which is why I think we just need to optimise some
routing, and suck it up.
Interestingly the AAPT/TPG Fibre400 services offer Unlimited internet at
400Mbps, but also say "subject to fair use" when you order it as a layer 3
Internet
Can’t say I’ve ever heard the words “fair use” and “business grade” being
applied to the same service before. If you’re selling it as an open 20meg pipe
then that’s what I’d expect to be getting if I was the client.
