What is your handoff. SIP or T1? I would go for T1 with SS7, but I have
seen carriers do crazy things now pushing their customers to where they want

I'm guessing the router is on IOS 12.4(24)T3 maybe?12.4(24)T2 has
24.3.2 and 12.4(24)T4 has 24.3.4.
If everything was working fine, reboot the router. Maybe you're facing
a bug in that IOS version or DSP firmware.
Did you maybe have a custom dsp image on the router flash (guido.dsp, etc)?
