This was being executed with the python datastax driver (I think it happened on both version 2.1.4 and 3.1.1, we were in the process of upgrading) but we were seeing this error in Cassandra's logs. Schema changes in C* are currently unfortunately inherently racy. It is thus up to the client to wait for schema agreement between schema changes (if they conflict at least, which they do in this case). In theory, this is handled under the hood by your client driver but that obviously a bit dependent of said driver.
