In most of my cases,
1. Booking reference number + First + Last name worked when printing at kiosk.
2. Passport just works at the drop baggage counter which provides
boarding passes.
3. For #2, just need to wait in queue, which took 5 minutes for me at
Montreal and 5 minutes at Mumbai.

On a bad day, your phone can decide to die. That's precisely what
happened to me while waiting on the airport lounge in the Buenos Aires
airport. Of course, went to the airline desk, and they were happy to
print a new set of boarding passes to me (after I identified, of
course). But some airlines might be less accomodating.

... and even with one. I once "trusted" that a phone-based boarding
pass would Just Work and proceeded through 2h+ of Chinese exit security
only to be told that they "couldn't stamp a phone" and I would have to
start all over again after returning with a paper version.

A special-case I'm sure, but Paul is right to imply that paper is at
least psychologically reassuring. :)

It's sometimes hard if you don't travel with a phone -

Basically the only thing you really need is your confirmation code, so
write that code down in a notebook (good practice to begin with :) ),
and the kiosks can print it for you fairly quickly and very reliably.
There is little to no wait at YUL right now, so it should be fine to
print at the airport (I just did!)

That doesn't reflect the reality. There is an official app for this, and
if you're an EU citizen you can use it. If you aren't (just as I am
not), the reality is that they print the boarding pass with no charge
but only if you've done the online check-in and are able to display the
PDF version of the boarding pass on any of your devices.

I'm flying with Ryanair all the time about a dozen times a year, and in
the last two years I think it was just twice that I had a printed
boarding pass when I came to the airport.

Untrue. They don't require printing boarding passes unless you need a
visa check, and they don't charge fees for printing at the airport
provided you've done a check-in and there are actual Ryanair employees
at the check-in stand. Airport employees though may charge you
separately, or choose to issue you a hand-written boarding pass.

It can be hard if you do travel with a phone too

Some airports insist that transit passengers pass through a bonus round
of security screening where you wait 15 minutes in a queue and then they
confiscate the duty free you acquired in the first airport

Having a printed boarding pass showing the time of your connecting
flight is extremely helpful for getting around those queues, it is hard
to create a sense of urgency by showing other queueing passengers a QR
code on your phone

introduce in-flight toilet fees[1].

Even those airlines that do still allow you to get a boarding pass at
the airport punish you for doing that with a long queue sometimes so the
5 minutes you lose sorting this out at home/hotel/conference/holiday
destination is the lesser evil.

For people who connect to a flight that is not on the same booking as
the flight out of Canada it is also very useful to get the boarding pass
for the second flight before boarding the first one.
