This seems to work.
I left last_successful_item in, since it's still needed to allow
skipping past menu items, and I understand how the seen_items hash is
supposed to be used now.

I don't experience this behaviour and don't remember that I
ever did. (Besides, I follow the tcpdump upstream mailing list
quite closely, and I think I would have noticed when such a bug
had been fixed... ;) )
