cups-filters. Evince converts documents to PS before sending them to
cups-filters, which avoids some of the bugs related to forms. I could
open a wishlist item for an optional PS-conversion as Evince does, but I

I've printed this file successfully with evince-gtk.
I do not have access to a real printer at this moment, but lp produces blank
page on cups-pdf printer, just like qpdfview. Evince prints to cups-pdf
successfully.

printing, especially of PDF forms, is a recurring problem. In several
cases the problem were related to the cups-filters or qpdf packages.
Have you checked whether printing the same forms works for Okular? Could
you try printing the affected file(s) via the lp command line tool?
