One thing that you need to make sure is that you target the specific OU
in AD that has the users. If you have multiple OUs with users then you
need to code it twice to go after both OUs.

 Please  explain  the  two dc= entries. I took dc= to mean the AD
 Domain  Controller. Not sure why there would be two dc= entries in a
 system with one domain controller.

dc in LDAPspeak stands for domain component; it is used to merge the
LDAP  namespace with the DNS namespace, sometimes called federation.
Therefore, the AD domain example.com corresponds to the LDAP container
dc=example,dc=com.  It's  not  related  to domain controllers. This is
certainly part of your problem.
