I just dist-upgraded to version 2.21.92-0ubuntu1. Now the symptoms are
different. In a way it's misbehaving more consistently, because
unplugging and replugging the AC power doesn't change the situation.
When the AC is plugged in it shows the battery icon without a power cord
and the tooltip says
The problem is no longer occurring in this form with the latest version
of the package (gnome-power-manager 2.21.92-0ubuntu1).

Did this problem appear only after the latest updates?
I have a similar problem, I log in on AC power, and it says, battery
discharging, 100%, AC power and displays icon though it shouldn't
