This is not a dupe of the firefox window decoration bug, as this has
been reported to apply to all windows. In any case, the reporter said
the problem went away after resetting compiz settings, so I'm closing
this bug. Please feel free to reopen if you continue to experience the
problem.

Does this just happen with Compiz turned on? This happened for me, but
only occurred when logging in. It didn't randomly happen during a
session.

Install the package fusion-icon and add it to your 'Sessions' menu. It's
a great little tool that will do what you require (I use it when using
google-earth as it doesn't seem to like Compiz at all.
