I got this crash on xenial 16.04 today when I tried to power off a USB
device that remained after I removed it from the system. I can reproduce
this on trusty as well on another Ubuntu system with trusty 14.04.4 LTS.

I've tried, but the proprietary nvidia driver can't enable the second
GPU if the first (the integrated Intel) is already working, so I'm left
with only one monitor and this glitch disappear (as it disappers if I
disable the intel GPU and use only the nvidia one with the nouveou
driver).
