Björn, unfortunately this is not true in any case. In LO 4.0.2.2 at
least, if I "Use the Wizard to create new Form" or I "Create a new
View", they both have the default main LO icon (that we use as
fallback), as their WM_CLASS is
WM_CLASS(STRING) = "soffice", "Soffice"
It should be libreoffice-ba
assuming this to be fixed by the bamf changes.

Are you still getting this?
DIalogs seems fine to me, but I see this problem with the "Database
Form", but this is actually a LO issue, since it matches the window with
"libreoffice-startcenter" class, while it should use "libreoffice-base"
instead.
