Depends on what you want to achieve from users of the form. If you
require users to fill in the correct address, and if that address is
already stored in a database, you might as well show hints of the
address and have the system suggest for auto fill into the fields for
your users.

For me, it has worked excellently from the beginning. Thanks to the
team who are behind this. I use my email to get notified about new

You could use CSS sprites where there is a difference in position of
an icon within the button image. For example if the icon or knob is
left edged, it could mean ON and if right edged then OFF.
Additionally you could add labels ON and OFF, also color such that
they are supportive alternative
