Regarding the empty divs, what about a post-processing step (like one
pass profiling, but on the other end) that processes the output and
turns this:

Not sure it justifies the extra markup, but in terms of what the xsls
do, one thing you get with a reference and refentries that you don't
with a chapter and sections is additional summary info (the refpurpose)
in the toc. You can see this in the toc for tdg under I. DocBook

for this if you're using a java xslt processor. In the following example,

I notice that simplesects are not chunks and so can be a way to let the
writer put subsections in an individual chunk/topic. Before we start
using them like this, I want to make sure I'm not relying on a bug. Is
it intentional that simplesects are not chunks?
One other thing I've noticed:
XMetaL can be scripted to do anything, but it doesn't have many friendly
DocBook macros out of the box. The included demo Journalist DTD is
similar to DocBook and those macros and features can be converted by
renaming some elements. XMLMind's XXE and Syntext's Serna offer much
better DocBook

Speaking of sidebars...using XEP 4.6, if I have a sidebar that would
extend past a page break, the sidebar disappears leaving a page
break/lots of empty space. I tried adding xsl:attribute

As I understand it, the ability to make a pdf commentable by Acrobat
Reader users (as opposed to full Acrobat users) is a proprietary thing
that Adobe controls. I.e. it's a feature they use to convince you to buy
Acrobat Pro or another Adobe product rather than using a free
alternative:
You can get something pretty cover-like just using titlepages for front covers
IMO. You can't do full page covers (e.g. with things that look like bleeds). If
you need something fancy like that you need to write some custom stuff at this
point. Here's a pretty old and kludgy example
