I'm trying to generate a custom title from an xml file. I've
followed the guides at:
http://www.sagehill.net/docbookxsl/HTMLTitlePage.html
http://www.sagehill.net/docbookxsl/TitlePageNewElems.html
http://www.dpawson.co.uk/docbook/styling/custom.html#TitlepageContent

For example:

Hi all,
We're trying to use mathml for some equations in some of our xml docs
but we keep getting 'failed to load external entities' errors.
mathmldb.dtd is installed locally but it doesn't seem to matter.
Is it not possible to embed mathml within a docbook (book or article)
file? Does it
Hi Mauritz,
Thanks for the reply. Sorry for the delay in replying. I didn't see
this until now.
Are you sure that there is an entry for the public or system ID for
dbmathml.dtd in the catalog file?
This was part of the problem. I had to add them to the catalog:

Hi Aankhen,
Woohoo! I figured it out. solution below.

Here’s my xsl file:

?xml version='1.0' encoding=utf-8?
xsl:stylesheet
xmlns:xsl=http://www.w3.org/1999/XSL/Transform;
xmlns:fo=http://www.w3.org/1999/XSL/Format; version=1.0
xsl:import
Hi Aankhen,

Thank you very much!
Glad I could help. :-) There’s something I don’t understand, though:
you say that you already do #3 for HTML output; why not do it for
XHTML as well? :-S Why do it this way? (Also, if you’re producing
XHTML, you probably want to close that ‘br’.)

Hi again all,
I've got a particular type of table that I want to set column widths
on via xsl rather than having to do it in the xml source.
The reason is that I have _lots_ of xml files that will use this type of
table and I don't want to have to maintain this if/when we change the
table
