Mystery solved! 11 of the 12 QSOs in question were for PJ2/W2UP. Apparently LOTW lumps them together, but I have a separate log for that operation. 1 of the 12 QSOs, S55W from 2004, was just recently uploaded by him. He had the mode as Phone, not SSB. I presume that mismatch is a problem
