Or possibly notifications are not making it across the wire. I would not be
surprised if the UDP notifications were getting dropped. If you click on
"Send/Receive" while the connection is "dropped" does the session
re-establish?

If so, one thing you could try is setting the clients to work offline but to
connect every few minutes, using the existing connection.

Not quite. Outlook does close the connection after a period of time. If new
mail is present the server informs the client via UDP (we've had this
discussion before). I forget what the default value was; maybe 15 minutes
like everything else. But it is a clean disconnection and reconnect is
seamless to the user. The problem description below fits the first part of
the behavior but the foobled connection state is suspect.

Outlook doesn't disconnect them due to time.  I suspect that there's some
glitch in the network connection during the four-plus-hour marathon session.
