Sounds like the MIME-type is definitely the culprit - given that
you're using ASP and getting a 404...

On Apache etc. you tend to be fine, but IIS is a pain - it just won't
serve things that you haven't set up a MIME-type for. I had this issue
last week; once set up, it's fine.
One way to test that - if you try browsing, manually,
the request for the flv is 404

Remove any spaces and check your case - that's the culprit in most cases.

When testing it works great.
