Known issue. page-number-citations are not handled properly in every
case. The width for elements representing forward references are only
known after the page where the reference lies has been resolved. The new
width is then hacked into the area tree representation. The adjustment

I think I've hit another bug with overlapping text and strange whitespace in
cross reference text.
