All normal, except for the crash when trying to save as .mif. 

To answer your questions:
 - Yes, you should be easily able to save as .mif. That is rarely a problem.
Feel free to e-mail it to me without graphics if you like.

 - Remember Missing Font Names is installation wide.
Opening, clicking OK to reformat with available fonts and then saving FM
files with Remember Missing Font Names OFF means that the substitutions are
permanent in the files saved. If the font becomes available (either by
installation or by file transfer to a system with the font available), those
files will not use the file, but the font substituted (if available :-).

 - Information in Book file
During book updates, the book file itself is updated with details from the
files.
This includes tags (paragraph, font, conditional text, colo(u)r view
settings and definitions, and a whole lot more.

book file — repeat, book file only. (Then when I then open the book 
file's files, they all open cleanly.) [FM10, Win7 SP3]

Any ideas about how to fix this (apart from creating a new book file)?

The crash on SaveAs mif is a bad sign. Yes, you should be able to saveAs MIF
and that should clean the file of information like this that is a holdover
from the previous state(s) of the book. The crash is indicative of some sort
of corruption.

The book file will store font information used in the files within it and
can maintain this information after fonts have been removed from these
files. That is cleaned up when it is saved as MIF and then opening and
saving the MIF as a fm book file. So it is probably a good idea to rebuild
the book.

Remember Missing Font Names is specific to the FrameMaker installation and
can be toggled off and on as needed. I usually leave it activated, for
obvious reasons. Since no fonts are actually used in a book file since
there is no text that will be printed from that container itself, it can't
have any effect on the printed output, but it will result in an entry in the
console window. That can be ignored.

That would be the solution, but it appears the book file is corrupt. Your best 
bet at this point is to recreate the book.
