Frame to pdf bookmark problem

I agree with Fred. This issue is almost certainly due to an issue with
either differently named text frames on master pages (for example Flow C for
chapter headings and Flow A for the main content flow), or text frames with
the same name that are disconnected.

If Special>Add disconnected page is used, this will results in this type of
issue.

Frame to pdf bookmark problem

Fred, it doesn't seem as straight forward as that. It only seems to
affect the first file in the book structure. I created 2 identical files
(all with the same text flows and connections). Whichever file was at
the top of the book structure, the problem as I described occurred, but
only for that file. Subsequent files where unaffected.
