You can't. PAM doesn't work that way.

I know that we can proxy a request with the specified ip-address if

the problem is can we proxy a request with the specified local port?
I've configured listen {type=proxy...} in radiusd.conf, but freeradius
proxy the request with
a random port.
What should I do?

Yes. Read raddb/sql.conf. Look for client. And see the NAS schema
shipped with the server.

Use a recent version of the server. This is supported in 2.1.x.
