We have had 2 reasons why we saw processing issues like this:

1. I recently found out on our VIP 5 they did not turn the level sensors on 
during install. These sensors are known to error so often, Sakura tells 
technicians to set the default to off. All the processor can sense is pressure 
and time. So it may be that the formalin is not filling all the way. You are 
noticing this on delayed runs, but on those runs are you often stacking trays? 
If so, isolate the cases in the upper trays and review. 

2. We found a clogged line. This happens with the formalin lines if you don’t 
do the hot water flush often enough. This can be an even bigger issue if you 
recycle and re-buffer your formalin (we do not).

We have terrible processing artifact if tissue sits in the formalin-filled 
retort (at ambient temperature) for too long (more than 10-12 hours) before a 
delayed process starts. The longer the wait, the worse it looks. We have Tissue 
Tek VIP 5 processors, and we process luminal gastrointestinal biopsies 
exclusively. I've attached some photomicrographs of problem cases on the 
Histonet Images website (with the same topic title). 
This artifact typically affects a few specimens per day (~2% or less), even 
though everything is done on the same processor; it may affect all tissue 
portions in a cassette or only some of the tissue in that cassette. Some tissue 
portions may only have the artifact on one half with the other half looking 
perfectly fine. The techs sometimes note the tissue feeling "crunchy" at the 
time of embedding and / or at the time of microtomy. These tissues tend to 
suffer greater chatter artifact and have trouble sticking to the slides. The 
sections look just as bad on recuts as the originals. Re-processing does not 
seem to help at all. 
If the cassettes sit in formalin in a container outside of the processor for 
days before the processor is loaded (with subsequent immediate start), things 
look perfectly fine. When we have staff around to start the processor 
immediately upon loading cassettes and empty immediately upon completion, the 
tissue looks perfectly fine. 

Our current processing program is as follows:

1. 10% Formalin, 30 minutes, ambient temp, p/v on 2. 10% Formalin, 30 minutes, 
ambient temp, p/v on 3. 65% Alcohol, 10 minutes, ambient temp, p/v on4. 80% 
Alcohol, 10 minutes, ambient temp, p/v on5. 95% Alcohol, 7 minutes, ambient 
temp, p/v on6. 95% Alcohol, 7 minutes, ambient temp, p/v on 7. 100% Alcohol, 10 
minutes, ambient temp, p/v on 8. 100% Alcohol, 10 minutes, ambient temp, p/v on 
9. Xylene, 15 minutes, ambient temp, p/v on 10. Xylene, 15 minutes, ambient 
temp, p/v on 11. Paraffin, 10 minutes, 60 degrees C, p/v on12. Paraffin, 10 
minutes, 60 degrees C, p/v on13. Paraffin, 15 minutes, 60 degrees C, p/v on14. 
Paraffin, 15 minutes, 60 degrees C, p/v on

Obviously there are times we absolutely need to use delayed start. I would 
greatly appreciate guidance, and I'll be happy to provide any other details 
that might be useful.

Hi Steven,

We ran into this issue and it was the pH of the water. We were using our 
sterile line from the facility but the pH was too high. We used tap and it 
worked fine. This will also effect your stock solutions if you make them 
yourself so be aware of that as well.

We pull the COA from the vendors website. Some are better than others but the 
are usually listed by lot number. I keep them in a network shared folder so 
they can be pulled by anyone that is asked for the evidence during inspection.

We use SteriCycle. We decant off all formalin and put the tissue in double red 
bagged biohazard boxes for incineration. The boxes are hauled by them and we 
have manifests in triplicate that are retained by our Safety department. Anyone 
signing the manifest needs to attend the training for the Mass DPH on how to 
handle haz-waste so you know what you are signing. We keep a binder of the 
manifests copies and we receive in the mail the original manifest stamped 
"destroyed" once that is done. We keep all records for three years. I don't 
know what other states require. I have a procedure to share if you want it.

We use our Acid alcohol solution from our AFB Stain. It pulls off the Eosin and 
leaves a faded Hematoxylin behind. We counterstain in Hematoxylin anyway so we 
find this protocol effective. Then we remove the deparaffinization step from 
our IHC protocol and run the slide.

[Histonet] Automated ihc staining of bone

When we have section lifting during IHC, we will use a concentrated Poly-L 
Lysine solution. You can dilute this solution but instead we will use a direct 
application (wet the tip of a gloved finger with the solution, and wipe down 
the slide, let dry) on one of our charged slides. This may cause a degree of 
counter stain background but always improves the IHC result for the 
pathologist. We use this for bone or heavily keratinized skin sections.

Re: [Histonet] best paraffin

Over all of my testing I found there are several factors to consider when 
choosing paraffin:

1. Processor type
2. Tissue type
3. Use - embedding  vs. processing

The processor type my dictate limitations on paraffin selection. For example we 
were told not to use Paraplast Plus in a processor that has a stainless steel 
retorts as the DSMO can ruin the surface over time.
The tissue type may dictate as infiltration is different through biopsies vs. 
fatty breast cases. Most do well with both but you may want to factor this in 
during your search.
The use is the biggest deal breaker when it comes to paraffin. If there is too 
much plastic vs. wax it will not infiltrate during processing. If there is not 
enough plastic it won't section well. 
Over the years I have had good luck with several products. I loved Paraplast 
Plus but when we upgraded our processor we had to switch because of the 
stainless steel retort. I will say, we were in search of a combination paraffin 
that we could use for both embedding and processing. 

We settled on the product from Polyscientific: Infiltrating/Embedding Paraffin 
Prills (Item Number: c827)

We love it.

[Histonet] IHC staining of cartilage

We use P8920 Sigma-Aldrich Poly-L-lysine solution. Great retention without 
excess background staining. You can prep a rack of slides in diluted solution 
but we put a gloved finger over the end of the bottle and and swipe it down the 
slide at full strength. Let the slide dry and move forward as you normally 
would.

are using and how many shifts you have. We have two stainers and each has their 
own log. Our stainers are connected to running water thus the drain lines that

Hi Eric,

We use a great product from Polyscientific, called "Infiltrating/Embedding 
Paraffin Prills" Item Number: c827 
https://www.polyrnd.com/product/infiltratingembedding-paraffin-prills/. I am in 
no way affiliated with this company so this is just a simple Histotech review. 
As the name suggests, it can actually be used for infiltration in processing 
and for embedding/microtomy.  I have used it at two different medical centers 
here in the Western Massachusetts area with great luck. I will tell you that we 
have Sakura processors and embedding centers. We use Leica RM2255  microtomes, 
and we do keep our blocks on ice while cutting. I hope this info helps.

When Fisher stopped producing The TissuePrep 2 Paraffin formulation last year, 
we were told by the Fisher rep that the new "Histoplast PE" was an equivalent 
replacement for the TissuePrep 2.  We have had nothing but irritation with this 
formulation with a magnitude increase in wrinkles and folds. In short, it is 
NOT an equivalent product.  Does anyone have any recommendations?  Is the 
"Histoplast LP" any better?

Hi Kelly,

We have tried several manufactures, as well as model types over the years. We 
have had sliding microtomes (Microm, no longer available), and rotary 
microtomes from, Leizt (old), AO (very old), and Leica. We have had several 
models from Leica and prefer their durability. If you are manual, you are hard 
pressed to find a more durable and reliable microtome than the "Leica RM2125 
RTS - The Essential Microtome" you can get the gross feed wheel in "American 
(toward you)" or "European (away from you)" so be careful to ask when ordering. 
If you are looking for Automatic we have 10 of the  RM2255 from Leica, which I 
think is now called "HistoCore AUTOCUT - Automated Rotary Microtome." These are 
great and several of my techs have adopted the use of a foot pedal to preserve 
their shoulders.  Good luck and happy shopping!

[Histonet] Paraffin Preference

Hi Paula,

I have worked in a couple different labs and in one lab we used Paraplast Plus. 
Because of the DMSO, tissue infiltration is superb with that paraffin. 
Unfortunately DMSO is not good for stainless steel so when we had to replace 
our processor (now with a stainless retort) we also had to replace our 
paraffin. We opted for Polyscientific Infiltrating/Embedding Paraffin Prills, 
Catalog Number: c827. It is a bit more money but after testing several other 
paraffins including the Richard Alan ones, this was the closest we could get to 
the performance of the Paraplast Plus. This is only the case if you are using 
one paraffin for BOTH infiltration/processing and embedding. If so, this 
paraffin is the best for the money. If you want to have two separate paraffins, 
one for processing only and a second type for embedding then your options are 
wide open. You can opt for paraffin where the chemistry has less plastics for 
processing and more for embedding, giving you a more supportive block.  In the 
lab I am in now we also use the Polyscientific for both processing and 
embedding.

We use charged slides and if a section falls off, we coat the recut slide with 
albumin to help get extra adherence. However sometimes even this does not help 
the tissue stick through the whole process (especially on GMS).


Does anyone have any additional suggestion we could try to get the sections to 
adhere better throughout the process?

[Histonet] Ventana Benchmark Ultra

Hello Warda,

As you know the Dako Autostainer and PT link set up works well but has a good 
amount of tech time involved in its use. We currently run the Dako as well as 
the Benchmark Ultra. The Ultra is a major upgrade in ease of use. The antigen 
retrieval is onboard with no tech time to switch and load after that step, like 
on the Dako. Our pathologist do still like specific antibodies run on the Dako 
platform but that is getting very specific with antibodies. They all agree they 
work on the Ultra but "like it better on the Dako." To me it comes down to open 
or closed. With the Ventana we use mostly prediluted antibodies with some 
exceptions where we make a prep kit. On the Dako which we have had for years 
longer than the Ventana, there was more room for developing the specific nuance 
of a given antibody. We seem to be able to tweak the settings on the Dako more 
to our pathologist liking. In summation the Dako is a bit more open but to me 
you cannot weight that against tech time and TAT of the Benchmark. We run the 
PD-L1 developed by Dako so we will always need that platform, but I prefer the 
Ventana equipment overall.

Hi Colleen,

We have all VIPs here at BMC but I have used the Leica in the past at a 
previous position. We currently have 3 - E300s, 3 - VIP 5s, and 2 - VIP 6s. The 
VIPs are great and really only need preventive stuff like O rings and 
lubrication. A service company for us Avantik takes great care of all of that. 
You can get refurbished VIPs at great prices and they will last!
