Ken,
   this really depends on the amount of support you get from your Medical 
Director. Any staple or sutures are removed at grossing by the PA or embedding 
or microtomy by the histology technician still affects the tissue. Either way 
the pathologist hasn't typically seen the tissue or slide yet. This is a fight 
that seems to go on everywhere that does larger specimens. We record it at 
microtomy as a quality issue of grossing if there is a staple or suture in the 
block just for documentation purposes. 
  The only way there can be can justification for the tech removing it is 
the PA time and blade that get ruined cost more than the HT that is embedding 
or doing microtomy. I'm not sure why it is not taken into consideration the 
PA's time and blade is already on the specimen in order to gross it so why 
should both budgets suffer. I wish you well on this fight, please share if you 
make good progress.

Hello All,

Recently we have had a marked increase in staples and sutures in our tissue 
that arrives for processing. I have tried to relay to our pathologists and PA's 
the difficulty in cutting sections and the tissue destruction in getting 
staples and sutures out once the tissue is processed to paraffin. I was told by 
some of them that in their experience histology should dealing with this issue. 
Though my own opinion is this is position is not valid, I've been asked to 
collect opinions from other histology departments if there are protocols that 
the grossing individual should be removing the foreign object. Please give me 
any feedback...Thanks

[Histonet] frozen section- workload and billing

Hi Histofolks,
   I need to pick yours brains...we are in the middle of building a 
workable computer system for our lab we have run into a hiccup when it come to 
frozen sections. As a tech I know there is actual hands on, stop what you are 
doing, do this now  work involved. My understanding from a billing perspective 
it is not billable workload but an interdepartmental consultation between 
surgery and pathology. The problem is how to build the system so we get labels 
for our frozen section slides that does not interfere with the billable 
workload that is. I was thinking maybe it should be built in the system the 
same way a control slide is, does anybody have any suggestions?

[Histonet] Can't find good help these days....

I graduated from a associates degree program in 1991 at a community college 
that has graduated an average of 6 students a year and the Mid-Atlantic market 
gets saturated with them. Every May these students are hungry for local jobs 
and many employers hire the newbies at a low salary (because they know they 
can), the students work at the first job long enough to get experience and 
their certification and move on to better paying jobs. I'm not sure what the 
solution is to fill the empty jobs in your area, but I do know there are 
histotechs looking for jobs out here.

It seems my message yesterday was a little cryptic at best. We currently have 
SunQuest CoPath Plus with Meditech as our hospital wide system. We are changing 
to Cerner hospital wide and PathNet for our lab (because we were just merged a 
couple of years with another healthcare system  they want us to change) My 
main concern is if Cerner's PathNet plays well with Vantage.

 I am hoping somebody can help me. Today was a horrid microtomy day, 
the static was brutal. I tried putting lotion on my hands, the fabric softener 
sheet, and wearing gloves but to no avail. I heard rumors somebody used to 
manufactor an anti-static brush but have yet to find one. Does anybody have any 
other tricks that could be shared?

To all you wonderful folks who responded, a big thanks! I've been doing histo 
since 1990 and NEVER had static like today, it's not usually that big of a 
problem in this mid-Atlantic state. When it does occur the tricks I named 
earlier usually help but I am always up for learning something new. I will try 
the low tech option of moist something in my clean-up tray tomorrow, first. I 
usually can't stand anything in my tray after facing but if given a choice I'll 
suck it up. I will keep you posted. Thanks again!

   if the power went out AND the backup generator , My question is under 
what circumstances are they expecting this to happen? That being said and not 
enough information, best answer would be send to another lab for processing to 
at least protect the integrity of the tissue for future testing until the 
electricity issue is resolved

No electricity means no lights, fume hoods, so hand processing is out and you 
should not let the tissue sit in solutions that might jeopardize future 
staining longer than recommendation (eg.Her2) and they should state any 
variances of normal process in the report.

[Histonet] Vantage patient tissue and + control slide labels

Hi folks,
 I am hoping to connect with somebody who has worked with Nexes (Ventana 
software),Vantage and CoPath. When the Vantage labels are printing we cannot 
tell the difference between the patient label and the positive control label. 
Our LIS wants us to use the exact staining protocol for our patients and 
controls but give a unique protocol number to the ones for positive controls. 
(Red flags are popping up all over for me on this idea)
What do other labs do to handle this?

I've see that Dako's DAB Away is composed of 1% Oxalic acid in water
instead other DAB Aways (like the one you are using) are composed of
sulfuric acid 10% in water.

I recommend a slide incubator and a metal cup or two. The paraffin dispenser we 
purchased seem to be a relicensed commercial coffee pot. The spigot keeps 
clogging because they did not design it in a way that it would stay warm so, 
the paraffin cools in it and clogs.

[Histonet] animal human biopsies

Some threw the thought of processing biopsies from veterinary clinics as a way 
of increasing outpatient volumes. I am concerned there may be a regulation

We run it at our lab, and our incredibly picky Hemepath is happy.  But our 
protocol is unusual.  We run extended CC1 and a 4 minute antibody incubation.  
We are using a large B cell lymphoma for our QC, which is working very well.

[Histonet] IHC antibody optimizing validating

Will you help me? I understand we are to use the known positives controls that 
the manufactures' recommends in the package insert when optimizing the stains, 
but I need to know what is your general procedure for optimizing (how many 
different staining protocols do you test) and validating a new antibody (how 
many different or known positive and negative tissues do you test [predictive 
markers I understand are 20])?

[Histonet] specimen marking ink

Hi Histo World, as I was cutting to day I was thinking why don't we see if we 
could get specimen marking ink directly from a tattoo vendor? When I first 
started in histo I was told the ink we use was actually tattoo ink. As we know 
as soon as somebody labels something as a medical supply the price is 
increased. Just a cost saving thought, I mentioned it to my immediate 
supervisor but she think it would be a liability issue. I thought we could 
test/validate it on skin tissue left over from a mastectomy or extremity. Any 
thoughts?

Good morning all,

To those working in the VA system, what LIS are you using? We are currently 
accessioning through Vista which if you work in the VA is used for many other 
processes such as timekeeping and ordering supplies but it is extremely archaic 
and not ideal for anatomic pathology, (in my humble opinion). Our barcoding is 
not interfaced with Vista and I am trying to convince management to upgrade to 
COPATH, WINDOWPATH, or POWERPATH, really anything other than what we have now. 
I have been told all VA Histology labs only use VISTA and I hope to God this is 
not true. So if anyone works at a VA, used to work there or know someone who 
works there, please contact me to let me know what you are using and how your 
process works. Thank you in advance.

Two things I have seen dealt with 1) distilled water filter had not been 
changed and was growing fungus 2) tech cutting slides had not changed water in

Hi Diana,
RE: In the past when we receive these samples there has been no issues cutting 
them.  Over the past few months there has been a tendency for these blocks to 
shred into fine strips when cut.  Almost like there was sand in the blocks.  
Mollifex, decal, warm or cold, doesn't matter they still shred.  Getting levels 
is such a challenge.  I have inquired at the collection site to determine if 
there is a change in the process of collecting that could cause this.  I have 
ensured the samples are not left on gauze or something to allow them to dry 
out. Even blocks with lots of blood and/or mucous will shred as well as blocks 
with only a scanty amount present.



There was a problem a few/many years ago with a paraffin manufacture using a 
cardboard conveyor belt for the pellets. Apparently, small pieces of cardboard 
making it's way in with the paraffin. You can check your melting tub and see if 
there is a tan residual in the bottom this will let you know if foreign 
material may be responsible for your problem.

Hi Valerie,
When I started in Histo in 90' everybody used the alcohol burners...Open 
flame concern became a concerned and the separate forcep warmers were purchase 
because the old embedding centers did not have the nice warmers like the new 
ones do. The last place I worked at had an old embedding center when I started 
but we weren't allowed open flames. Fortunately, we found an unused Bacteria 
Incinerator that Micro. wasn't using and used that until that embedding center 
died. That worked great!

[Histonet] The Good Old Days...

I enjoy hearing sincere reminiscing...Even though us kids don't know how good 
we have it, some of us enjoy having an old tech beside us on the bench. I 
find weeding through the sarcasm can be profitable and in doing so have learned 
so much. What the old techs did on a daily basis, we only did in the practice 
lab and when the automated instruments and pre-made solutions that we have come 
to rely on fail, experience is so very valuable. Only by their blood, sweat and 
tears have we benefitted however, we have so far to go, let's do it together.

[Histonet] Sakura Prisma stainer/film coverslipper

Hi Haley, we've been using this H  E stainer and tape coverslipper for about 5 
years with pretty good results even though we do not have the piece to link the 
two together. The very few problems we have had are related to lack of daily 
maintenance which is minimal. We are in the process of handing it down to 
cytology and trying to use the new Symphony for H E because of the tracking 
system option. The folks that do the microtomy are not happy because using the 
trays are more time consuming due to the learning curve (we are told), which 
is the trade-off. I hope this helps.

I am trying to decide on what equipment to purchase. I have picked the 
processor, microtomes, embedding centers and a few other pieces, but now I 
still need to decide on the HE Stainer and the immunostainer. My options I 
have in front of me are for the HE stainer are Sakura Prisma stainer/film 
coverslipper and the Leica ST5020-CV5030 stainer/glass coverslipper. For the 
Immunostainer, I am looking at the Dako Autostainer and the Leica Bond Max. Any

[Histonet] IHC tissue falling off.

Hi CJ,
 we had this problem the year before last, it turned out to be the slides. 
Apparently, the manufactor had a couple of  bad runs. We switched to Avantik 
Biogroup Ultra Bond and haven't had a problem since. The bonus was they were 
more reasonably priced compare to our contract vendor. Send me a e-mail if

After the detergent treatment, rinse well in distilled water and dry in the 
oven.
Dip in clearant and coverslip.

We do not treat our developed IHC slides in alcohols or clearant due to 
potential loss of some labeling - not DAB.

Recently my lab has been experiencing a significant occurrence of tissue 
falling off our IHC slides. We are looking at a variety a factors and I have 
been watching how some technicians dehydrate their slides after staining. We 
are running Ventana IHC stainers, so we remove the oil based liquid coverslip 
in a weak solution of Dawn and then run the slides down alcohols into xylene. 
Some technicians are very aggressive in their agitation of the slides. It is 
almost comical how much sloshing around in the reagents they do. How can I get 
these people to ease up? Does anyone have any protocols on how many dips it 
takes in each reagent to properly dehydrate slides?

Also, some of the other issues we are looking at causing tissue falling off the 
slides are; tapping slides to remove water, not double-dipping slides, lotion 
on hands, over temperature, and the brand of charged slides we are using. Can 
anyone think of any other factors to look at?

accessories of the cryostats. I hate when they move things on websites but I 
guess they are employing the grocery store theory (move thing so customers will 
see other stuff to buy as they look for what they really need)

My experience with the square  rectangle ones, they are ok for about 1-2 uses 
with the paraffin, then the centers warp. We did freeze some of our case tissue 
to save for long term storage in the square ones with OTC but we didn't have to

I found an old box of Tissue Tek  4728 cryomolds in our Mohs lab.  They are 
round with a small tab on the side.  We like them so much better than the 
disposable embedding molds sold for paraffin embedding.  I have seen cryomolds 
that look like the same ones for paraffin but are they any thinner for quicker 
freezing?

1. Does anyone know where to get the round ones?I do not see them on the 
Sakura website.
2. Do the square ones that look like our paraffin molds release easily?

If the tiny bubbles are a new thing it might be humidity causing tiny water 
droplets to condense in your melted paraffin or on the paraffin pellets/flakes 
in transport to your lab or in your lab that show up after melting.

RE: too dark AFB blue counterstain

I don't know the auto-stainer you use or the protocol you follow after staining 
but might not be the machine. If you dehydrate  clear your slides by hand 
after staining it might be the alcohols. Some techs are known to use absolute 
alcohol where the 95% is supposed to be in dehydrating before clearing steps. 
If that is the case, not as much of the blue is removed during the process.

Hematology does a cytospin, we fix it in 95%Reagent alcohol for 10 minutes then 
do the iron stain on the NEXES automatic special stainer. No complaints.

[Histonet] bone marrow aspirate slides for iron stains

Try hydrating the slide in 95% then distilled before staining. The iron stain 
is made in water so hydrating to water should help the stain transfer better.

While I have sectioned a lot of different stuff, I have not done mouse brain
before and I am having considerable difficulty. ??The brains are fixed and
were embedded by our pathology department on their automated system. ??When
cutting, the paraffin cuts nicely but the brain tissue shreds as if the
infiltration was insufficient. ??I have re-embedded one, just melted and new
paraffin then vacuum oven for 2 hours. ??The results were the same. ??I don't
know if it would help to re-embed going back to EtOH. ??Also, from my distant
past I recall that when we were paraffin sectioning whole human larynges,
that the face of the block was soaked by placing a wet gauze pad on it. ??I
know this help but don't remember what the solution was on the pad. ??Any
help would be appreciated.

What might that process be?  How long should it take to get a good histotech

What do other labs' training processes look like compared to ours?  Is there 
any place that trains outside techs for such a circumstance?

Do you have national contacts that may know someone who knows someone, etc.?

RE: [Histonet] eosin in the processor

Hi Kim, 

try 10cc of the prediluted Eosin (you use in H  E staining) in your last 100% 
before your Xylene on the processor. By putting it in the last absolute it will 
not wash out. Your first Xylene will end up a little pink but your eyes will 
not strain as much trying to fine the tiny tissue fragments. (great for 
thread-like prostate biopsies too) We had been using it for years  our IHC 
seems fine. Make sure to keep your solutions for the clean cycles maintenanced 
on the automatic processors to avoid clogging. The only reason we stopped was 
the folks we purchased the new processor from told us using it would invalidate 
the warranty on the new processor.(I really miss it)

was the 2nd 100% ethanol).  Also, back then my lab didn't do any IHC; will the 
eosin affect any IHC that might be done (I am guessing no, but I want to be 
sure).

Hi all,
I am performing IHC on tissue that was formalin fixed for 48 hours and then 
embedded in OCT.  I have been using Gold Plus slides from Thermo Fisher.I 
am staining rat spleen and mandibular lymph nodes.  With a peroxidase method, 
the tissue completely digests off the slides as soon as I apply the peroxidase 
block (looks like pouring hydrogen peroxide on a cut - just turns white and 
bubbles off of the slide).  I have tried various concentrations of peroxidase 
block with no luck.  With the alkaline phos method, the tissue is staying 
adhered to the slide, but I am getting significant 'tissue lifting' which makes 
the pathologists interpretation very difficult.  Is there anyone out there with 
experience in doing IHC on formalin fixed frozen sections that has experienced 
similar 'tissue lifting' problems?  I have ran this antibody on this tissue 
with immunofluorescence successfully - that may be the only choice we have for 
this particular tissue.  Would really like to be able to perform this staining 
with either an HRP or AP method for light microscopy if possible.
