[Histonet] Validation of new G2 Coverslipper

Good Morning Histo Peeps:

We just purchased a new Sakura G2 Coverslipper for our IHC Lab.  The engineer 
will be coming Monday to make sure all adjustments are in alignment and fine 
tune anything which might have gotten out of alignment due to shipping. 

I have never validated a new Coverslipper in any of the labs I have managed.  I 
searched the internet and was not able to find any information or instructions 
pertaining to Coverslipper Validation.  I am of course going to run several 
test slides prior to running any actual cases on it.  I will instruct my staff 
to date and provide test information on the slides to accompany the paperwork 
Sakura provides for the set-up of the equipment, but I am not sure that will be 
sufficient. 

Since we our a research facility, my IHC Director does not feel we need to 
validate our new Coverslipper to comply to GLP.  I just want to make sure for 
my own assurance.  I would greatly appreciate any guidelines for validation you 
may want to share with me.

[Histonet] GLP lot# documentation

Hello Histo Land:

I am the new IHC manager for a research lab which works primarily with animal 
tissue and we are moving towards GLP compliance for our department. We have an 
extremely large client base which consists of Big Pharma, private research 
companies, universities, etc…  We are in the process of updating our SOW and 
SOP forms, Stain line maintenance forms, solution stainer change logs, etc…  

I have a question regarding documenting lot #’s for our hematoxylin 
counterstain protocol for IHC's.  Do any of you who are GLP compliant document 
lot numbers for your alcohols and xylenes?  Our director and QA officer would 
like to incorporate documenting lot numbers for alcohols and xylenes.  Our 
histology department who is GLP certified only documents their lot numbers for 
Hematoxylin, Eosin, Bluing solution and High Def solutions for their H stain 
line.  They currently do not document lot numbers for  alcohols and xylenes.   
I have primarily worked for Clinical pathology labs, who did not document lot 
numbers for alcohols or xylenes.  Please let me know if you do.

[Histonet] Bx Processing run

Good morning Histopeeps:

I am reaching out to you this morning to ask for your processing protocols for 
your biopsies and routines.  Our department has two 25 year old VIP Tissue 
processors.  Prior to my arrival, the department was only using one processor 
at a time and running the biopsies and routine tissues together.  They were 
also only rotating solutions once a week even though there were close to 300 
cassettes a day.

With that being said, we are now using both tissue processors and separating 
biopsies from routines and we rotate all solutions daily.  The specimens are 
put on the machines at 5:00 PM.  This is not an option.  Our biopsies finish at 
5:00 AM and the routines finish at 6:30 AM.  I would like to have our biopsies 
finish by 3:00 AM and the routines finish by 5:00 AM.  Naturally, I do not want 
to compromise the quality of our tissue samples.  I would appreciate you 
sharing your processing protocols with me which would include solutions and 
time in each station.

[Histonet] Leica's Spectra w/ coverslipper

Good morning Histopeeps:

We will be receiving our new Spectra H Stainer w/ cover slipper the week of 
May 4th.  This instrument was in negotiations for purchase prior to me coming 
on board.  I have worked with the older models of Leica’s H Stainers, but am 
unfamiliar with this one.  Could any of you who has this unit be willing to 
share your thoughts with me.  It’s been several years since I have validated a 
new H stainer.  Do I need to validate the instrument along with validating 
and optimizing the H stains?

[Histonet] histologists assisting with transplants

Just curious, this is definitely a 1st for me.  Received a call this morning 
from my histotech who is on call this week that she was called in at 3:30 AM to 
assist one our pathologists with a liver transplant.  In all my years working 
in this field, histologists have never had anything to do with transplants.  
Have any of you out there had anything to do with transplants?

[Histonet] Tissue disposal

Good morning Histopeeps! After reviewing our Policy Procedure Manual, our 
manager is questioning the bags we are using for disposing our tissues after 
diagnosis is complete.  We are separating the tissue from the formalin by 
straining the excess formalin and neutralizing it.  We then put the tissue into 
“Large Biohazard Bags” and the EVS staff picks it up and disposes it.  My 
manager is wondering if the “Biohazard Bags" are appropriate.  Should the bags 
state “Tissue” on the them?  Are there any special disposal bags available?  

While working for a GI Lab in Monterey, CA we put all our bottle and tissue in 
"Biohazard Bags" and a Hazardous Waste company picked up the waste products 
from our endoscopy department along with ours.  Any comments and suggestions 
would be greatly appreciated.

We recently brought on an Ophthalmology Pathologist from MD Anderson and they 
use histogel for orienting their thin eye specimens and need a microwave to 
heat it up.  We do not have a microwave in the lab for their use but I had 
ordered one for them.  I just found out they used the microwave in our lab 
lounge to heat up histonet which was opened and being stored in our gross area 
refrigerator.  

Do any of you know the CAP regulation against use of laboratory reagents being 
used in a microwave being used for food consumption?  I ended up removing the 
microwave they used and put it in the histology lab.  Our Director wants me to 
write a formal letter to submit to him.

Ask Biocare or Dako.  Unbeknownst to most of the public, it is the same exact 
unit they had years ago.  Lab Vision made all 3 units. Their brand, Biocare and 
Dako.  Lab Vision just put different branding on the various units when they 
were sold.

[Histonet] Histo techs assisting on BM Bx's

Good Morning Histo Peeps:
Back from a great trip to NSH NOLA and hit the ground running yesterday.  I am 
faced with a department coverage issue which impacts our TAT.  I am curious how 
many facilities have their histologists assist with BM Bx’s.  I have never in 
my 50 years as a histologist assisted with BM Bx’s.  That responsibility was 
under the hematology department responsibilities.

We have 4 FT Histo tech’s and 1 PNR in our department.  Yesterday we had 2 
histologists pulled away from bench duties to assist with BM Bx’s, and then 
another histo tech was called to go to the main hospital to cut FS’s, which is 
2 blokes away.  One of the tech’s was gone for 3 hrs because the pathologist 
performing the BM was extremely slow.  That left 1 tech and myself to cover 
bench duties.  

Keep in mind this is a county hospital with limited funding and a freeze on 
staffing.

[Histonet] Disposal of contaminated gloves, paper towels etc... after grossing

Good morning Histo Peeps:

I am curious as to your laboratory/hospital policies on how to dispose of 
contaminated gloves, paper towels etc… after the PA/pathologists gross a 
specimen.  

Often times we receive fresh tissue in the lab and we do not know if the 
patient has TB, Hepatitis, HIV, or any other infectious disease.  I was always 
taught that to treat these items universally as potential biohazards.  Do you 
discard these items in a Biohazard Waste Container,, or do you discard them in 
a regular trash basket?

[Histonet] Plus Slides

Good morning Histo Peeps!

I would like to get information on where you are getting your Plus Slides for 
IHC.  We are currently getting our (+) charged slides from Cardinal Health and 
they have little or no charge.  We currently have 2 Ventana IHC Instruments, as 
well a Special Stainer.  We are having tissues falling partially and completely 
off even on the SS Stainer. Also, I realize Ventana will not back the IHC 
results unless the end user uses a particular brand of (+) charged slides.  

Years ago when I worked at Biocare Medical I used to QC our Kling-On Slides 
before selling them by checking to see if the sections indeed stuck to the 
slides without any lifting on each lot.  I realize that positive charged slides 
have an expiration life, and these slides are newer lot numbers within their 
expiration.

[Histonet] Stain removal on clothing

Just found this info on-line for stain removal.  I assume it originally came 
from Chuck Churukian since it’s from University of Rochester.


REMOVING STAINS FROM HANDS AND CLOTHING

Some stains can be removed form the hands by using *Erado-Sol.  Dilute Chlorox 
(or similar bleach) and Erado-Sol are useful for removing stains from white 
clothing.  Care should be exercised in removing stains from colored clothing.

To remove the following stains use the solutions indicated.

1.Alcian blue - to remove from the hands use the acid alcohol solution. 
 For cleaning glassware try using dilute Chlorox or acid alcohol.  If the stain 
resists, use them alternatively.
2.Auramine-rhodamine - acid alcohol.
3.Basic fuchsin - Erado-Sol.
4.Carmine - acid alcohol.
5.Chromic acid - concentrated aqueous sodium thiosulfate containing a 
few drops of sulfuric acid.
6.Crystal violet - Erado-Sol or Chlorox.
7.Eosin Y - saturated aqueous lithium carbonate.
8.Fast green and similar acid dyes - saturated aqueous lithium 
carbonate.
9.Hematoxylin (alum and iron) - Erado-Sol or acid alcohol.
10.  Iodine - 5% sodium thiosulfate.
11.  Methylene blue - acid alcohol, Erado-Sol or Chlorox.
12.  Nuclear fast red - acid alcohol.
13.  Picric acid - saturated aqueous lithium carbonate.
14.  Potassium permanganate - 5% oxalic acid.
15.  Schiffs solution - Erado-Sol or Chlorox.
16.  Silver deposits - Lugols iodine followed by 5% sodium thiosulfate.
17.  Sudan stains (Oil red O, Sudan black B) - acetone.
18.  Thioflavin T - acid alcohol.

[Histonet] Alcian Blue stain on lab coats

the stains out.  I have tried oxy clean incorporated with bleach, as well as

Our lab has a formalin-free environment with new Milestone microwave hybrid 
tissue processing technology, Leica 2255 microtomes, a new Leica embedding

ATTENTION HISTOLOGISTS! Do you know of anyone who is looking for a job? We have 
an opening in our pathology laboratory located in a private practice 
Gastroenterology Medical Group in beautiful Monterey, CA.  It is a great 
opportunity for someone who is HT or HTL certified. The candidate must also 
meet CLIA grossing regulations. 

Our lab has a formalin-free environment with new Milestone microwave hybrid 
tissue processing technology, Leica 2255 Microtome's, New Leica Embedding 
Center and Leica H Stainer.

GI experience helpful, but we will train a knowledgeable and willing candidate. 
We insist on quality embedding and cutting skills. Duties to include (but not 
limited to): accessioning, data entry into our AP Easy LIS System, grossing, 
embedding, microtomy, limited manual special stains, manual coverslipping, 
general histology and histology related clerical duties. The right candidate is 
reliable, self-motivated, articulate and well versed with anatomy 
identifications. Must be adept with Microsoft, Excel, and having excellent 
typing skills.

Full Time Days, Monday through Friday with NO WEEKEND’S!  We offer A benefits 
package including medical, dental and vision insurance, paid holidays, 
vacation, sick accrual and a profit sharing 401K plan.  Salary is negotiable

We are CLIA/CMS certified only.  My lab assistant only has a high school degree 
and never had any background in any sciences. She knows nothing about 
trouble-shooting, nor theory and practice. She has worked primarily in clerical 
roles for our GI practice for 13 years. In the past 1.5 years, I trained her to 
accession, process, embed, cut, stain, and coverslip. We do not do IHC here. 
Our CEO/GI, MD said she could oversee our new HTL during my absence.

into a situation where a OJT histology assistant supervises an HTL when the 
histology manager is absent?  Is it even legal?  Any feedback would be greatly

We keep ours for one week after final report. There have been past issues as to 
tissues  submitted from our endoscopy center not matching up to our gross 
exams. Gastric tissue included with esophageal tissue, I account for every 
MINUTE fragment, so there were absolutely floaters from somewhere!. We account 
for everything when I gross the tissue!  I have a embedding log sheet which 
accounts for every piece submitted, as well as who embeds it, cuts, it, etc.. 
as well as any comments which just don’t match up to the gross.

Tim: I know full well about that facility and have known about their activity 
for several years!  I am at a point in my life I really could care less about 
any consequences! I am Too old to care about being PC when it comes to the care 
and nurturing of histologists! I am 67 and all about patient care and what we 
do to do our best to produce the best quality we can!  What can anyone do to me 
at this point in my life???

[Histonet] Cassette Printers

Good morning Histoland! 

Our department is looking into purchasing a new cassette printer which has the 
capability of interfacing with our AP Easy LIS System. I am currently demoing a 
General Data ID/Positive CL-01 Laser Cassette Printer and label printer system. 
We are a fairly small lab with limited space so a small foot print is 
essential.  We also do not need a multiple magazine. 

I would love to hear your comments, good, bad or indifferent…

[Histonet] RIP Wanda Platt Jones

My feelings for Wanda Platt Jones: Although I am Buddhist, and believe in 
reincarnation, I am untuned to all religions! In Wanda's case, perhaps, she was 
needed to be with her GOD to help serve in the best way possible for the 
ultimate good of everyone. She had an energy and presence which was focused 
unconditionally for true GOOD and LOVE for everyone. 
Wanda to me is a True Angel; who now is looking down on us, and helping us to 
become better human beings. Anyone she came in contact with benefitted from her 
remarkable love, sweetness, and acceptance. I will truly miss her, but I know 
she has not left us. She is hugging and guiding us, and showering us with her 
love and sweetness. 
She is sitting in a Prestigious Histology Chair in the Sky with all the Greats, 
such as the likes of Chuck Churukian. He was another Great Christian 
Histologist, and a Very, Dear Friend! XoXo

We monitor room temp and record it as per CAP, and CLIA.  Are you sure the 
request wasn’t for monitoring room temp because of paraffin blocks, not slides? 
The concern is for paraffin blocks melting, due to excessive heat.

GP’s are fine for general health issues, but I would certainly get more 
conclusive tests done by a neurologist, as well as contacting the best worker’s 
comp attorney in your area who has dealt with chemical exposure cases.

Studies show we are #1 in this country for the most hazardous professions. It's 
safe compared to when I 1st started in this field in 1965! Well, I am a 
dinosaur from back in the day when we had inadequate ventilation, a shortage of 
fume hoods, inadequate education on hazards, safety and PPE's. 

In 1979 I started to work at OHSU in the surgical path lab, as well as doing 
research projects. We made up all of our own HE's and special stains from 
scratch, as well as made up our own 10% NBF in 55 gallon drums without fume 
hoods, ventilation, masks or gloves. I also worked with Glyco-Methacrylate 
embedded tissues without hoods or gloves! Since it was a medical school, we did 
every special stain under the sun and dealt with about every chemical, reagent, 
acid and stain you could think of! We also smoked cigarettes and drank coffee 
in the lab while we embedded and cut! We sure were a naive group back then!

In the early days, the facilities I worked in never had MSDS information 
available. In 1989, while at Emanuel Hospital, Portland, OR, I researched and 
compiled the MSDS information on all the chemicals, reagents, acids and stains 
that we used. That was the 1st eye opener to what me and my fellow histologists 
dealt with on a daily basis. 

In 1988 I had base line tests done because I was having issues with dizziness, 
balance, reflexes, and short term memory loss. In 1992, I had extensive 
neurological tests done, as well as a sural nerve bx taken from my right ankle. 
It was found that I had nerve damage, loss of balance, no reflexes, numbness in 
my fingers, hyper sensitivity and reduced feeling on my right side, hearing 
loss in my left ear, and an aedes pupil in my right eye. It was concluded these 
were the results from exposure to multiple toxic chemicals in an extremely 
small room with excessive heat and NO VENTILATION at current hospital I was 
working at. These health issues are irreversible. I just deal with it. I was 
the 1st person who won a case for this in the state of Oregon, but it had 
consequences. I won the battle, but lost the war!  

I now have Primary Biliary Cirrhosis (PBC).  I was 1st diagnosed in 2008 for 
PBC. I think this condition was caused by my continual exposure to Multiple 
Toxic Chemicals. You may, or may not agree. A huge amount of the chemicals we 
deal with in the histology lab targets the liver and is absorbed through the 
skin or is inhaled. Here is the link for PBC.

 Irene said this: CLL cells changed in character so Chuck began chemo 
 treatments on Dec. 1. His second one was Dec. 21... He has six altogether 
 -The first cycle was difficult... he was very weak-in bed most of the time. 
 He is now improving-thankfully. We are praying that by April, he will be in 
 remission after all the treatments are completed. RIP Chuck Xo

 co-worker, when she was pregnant wasn't allowed to gross (hence contact with 
 formalin). Because of the NASH I get blood tests done every 6 months. It will 
 be interesting to see if they have changed possibly due to the exposure to 
 xylene, which I haven't had exposure to for years otherwise.

 This is very interesting.  I have worked in the field for over 20 yrs, and 
 was pregnant while working in a small lab.  Mostly everything was manual, and 
 I did not use all of the safety precautions I should have (my fault). My son 
 had severe speech and language delay as well as a language processing issue 
 when he was smaller, and now stutters.  I have often wondered (as most 
 parents would) if my lack of precautions may have contributed. We stress the 
 safety issues to the students daily, to get it all in their heads.  I often 
 tell  people that I am so happy because I work with xylene, and I feel real 
 good.  So far, I don't see any noticeable symptoms of anything wrong, just 
 the usual-decreased olfactory sensing, repetitive motion issues, plantar 
 fasciitis, and varicose veins.  Hair loss was an issue at one time, but that 
 was attributed to PCOS.  Now that I am not in the lab full time anymore it 
 has decreased since I went natural (I'm African-american).  One thing I have 
 noticed is my C-reactive protein is high, and I take a statin for that.  I 
 have no liver issues, and my functions appear to be normal.  Next time I go 
 in for a physical, I will have my workplace hazards documented in my EMR.
 I will look up Primary Biliary Cirrhosis (PBC), to get some more information, 
 especially since I was born jaundiced and I am anemic.

Just an FYI- We do not have a LIS System!  The GI CEO and administrator DOES 
NOT want to invest in a LIS System! Our pathologists are contracted, so they 
just go with the flow… which is difficult for me to have support to upgrade our 
system.

As usual, we are an after thought! When I came here 4 years ago, I thought I 
was in a time machine and had been transported back to 1989 before they had lab 
computers or LIS Systems! We are currently using “PCM”.  It is geared towards 
GI MD’s and their practice. It is a system which is used for scheduling and all 
patient information. It interfaces with the billing system. This system was in 
place before I got here.  

Currently, my assistant has to create a Path Report and type in gross, etc… 
then the pathologist types in his microscopic findings and sign’s the report. 
Everything is hard copy….. I have basically time warped to before the 90’s when 
I created all these workload recording forms up.

We have hard copy req forms which are hand written by the nurses in endoscopy 
with all the patient and specimen information.  We do have an automated 
cassette printer, but we hand write slides.   We hand write an embedding log 
with # of number of pieces in the cassette, etc…for QC purposes.  I hand write 
CPT charges on a hard copy and submit to the billing dept.  Nothing is bar 
coded, etc…. It is better now that I have an assistant. 

The endoscopy center has upgraded to “Provation”. It interfaces with our 
current “PCM” (Prime Clinical Management System) which is antiquated!  The new 
computer system for scheduling and billing is Athena Healthcare” it is geared 
for billing and appointment scheduling etc We are also interfaced with 
Elkay?  and are in training now and go live August 11th! 

We are still going to have to do the same procedure with pathology as usual, 
but we have to create the templates again!  They are not getting us a LIS 
system to interface! 

Oh wait, I’ve been a histologist for 50 years and still going strong!  It’s 
frustrating, but at least I live in a beautiful area!

[Histonet] Athena Healthcare computer system

Just curious if anyone out there is using the Athena Healthcare Computer system 
for your AP lab. If so, do have any special Macros you have created, or do you

[Histonet] Athena Healthcare computer system

Good Morning and Happy Friday Histoland! 
Just curious if anyone out there is using the Athena Healthcare Computer system 
for your AP lab. If so, do you have any special Macros you have created, or do 
you use a separate LIS system? From my research, it appears to be a billing a 
patient scheduling system. I would greatly appreciate any feedback you can 
provide.

[Histonet] P26 billing for IHC Professional component

Good morning Histoland:

Happy Friday! I was asked by a friend to post this on histonet.  They have a 
question for those of you who may be familiar with billing for the IHC 
professional component only.  

Here is the scenario; a small lab who does not perform IHC in-house sends a 
block to be cut and stained for IHC to a reference lab who performs a stain 
only IHC. The reference lab then sends the stained slides back to the ordering 
facility and their pathologist makes the interpretation of the stained slides.

Arrangements have been made for the reference lab to charge the patients 
insurance company and/or patient for the technical component of the IHC stain.  
How would the requesting facility billing department charge for the 
pathologists professional component? Do they charge with a P26 CPT code only?

[Histonet] Levels for Gastic and Esophogeal Bx's

Good morning Histoland:

Happy Friday!  I believe this question was asked last year, but I would like to 
revisit this again. I have worked in a variety of laboratories in my career, 
both small and large. Most of my experience was in Oregon with large hospital 
laboratories and University institutions. Back then, we cut (6) levels on all 
our bx’s, and picked-up (2) sections on each level.  My current facility is 
cutting (3) levels and picking up (1) section on each level.

I would like to ask what everyone else is doing.  The reason I am asking is 
because our pathology director is requesting a change in our protocol to cut 
only (2) levels and (1) section at each level.

Speaking for myself and the labs I have managed, I have been checking ALL temps 
and recording them since the early 90’s. These include waters baths for tissue 
flotation, and ones used for special stains, ovens, refrigerators, freezers, 
cryostats, tissue processors, embedding centers,  decloaking chambers and room 
temps.

 research microscopy group, then, for a short period, product development

I have a Milestone Microwave Tissue Processor and use Prowave from Milestone. 
It’s proprietary, but it has alcohol in it and does a great job on our tissue.I 
haven’t used mineral oil for years!
