Hi If you are using buffered Formaline so the Eosin color will not resist until 
the end of tissue processing.I recommend to add small amount of stock Eosin to 
the last alcohol in theTissue processor.

Yes this is possible. just don't use a too strong concentration. The eosin
should give a slight pink color on the tissue after processing and after
embedding.

Annual Result Comparison



 

For immunohistochemical and FISH/ISH tests that provide independent
predictive information, the laboratory at least annually compares its
patient results with published benchmarks, and evaluates interobserver
variability among the pathologists in the laboratory.

NOTE: Individuals interpreting the assay must also have their concordance
compared with each other and this concordance should also be at least 95%.

With specific reference to estrogen and progesterone receptor studies: in
general, the overall proportion of ER-negative breast cancers (invasive and
DCIS) should not exceed 30%. The proportion is somewhat lower in
postmenopausal than premenopausal women (approximately 20% vs. 35%). The
proportion is considerably lower in well-differentiated carcinomas (<10%)
and certain special types of invasive carcinomas (<10% in lobular, tubular,
and mucinous types). The proportion of PgR-negative cases is 10-15% higher
than for ER-negative in each of these settings. Investigation is warranted
if the proportion of negative cases is significantly lower in any of these
settings.

I didn't receive any responses in regards to my last post so I thought I would 
try one more time.  Does anyone have a competency or orientation checklist that 
they use for lead techs or supervisors that they would be willing to share?  On 
my last post, everyone that messaged asked for copies of my assessment and the 
assessments that were sent to me, which I did send to everyone that requested 
them.  If anyone else would like me to send my assessment to the them to look 
at, please let me know and I will be happy to share, plus I would be 
appreciative of any feedback- positive or negative- and any suggestions for 
improvement.  I tried to send an attachment of my assessment to the histonet 
but it kept saying it was waiting for moderator approval and it never was sent.

Struggling to find an answer.  We do a lot of GI biopsies in our lab.  
Sometimes they look wonderful without any nuclear bubbling, other times the 
bubbling is pretty intense.  Since nuclear bubbling is often attributed to 
incomplete fixation we of course have investigated the fixation times.  I do 
not find that the problem is fixation.  In fact some of the biopsies end up 
fixing for 48 hrs before processing. (weekend).  There was a suggestion last 
week or so that there might be water trapped under the slides after cutting and 
before staining.  I really thought that this might be the issue however I'm not 
sure at this point.  Extra drying seems to help but sometimes slides side by 
side are so variable, one with bubbles and one without.  I also don't believe 
the problem is in the processing schedule since the problem has shown up on 
both a rapid and a normal schedule. (therefore longer dehydration, clearing, 
etc.)

I am wondering if anyone else has worked with this issue.  Here are my 
questions:


1.        Could it be something that is happening with the tissue before it 
gets to the lab?  Usually a delay if fixation  causes other artifacts but not 
bubbling.  Could it be heat from the GI procedure?

2.      We do use blue sponges for our biopsies.  I know some say get rid of 
the sponges but has anyone seen this problem caused by usage of sponges?

3.      What about the heat stage in our Prisma stainer?


I am really getting frustrated.  Pathologists never complain however I would 
rather all of the tissue did not have the "nuclear bubbling".  Again we only do 
biopsies so I really don't think the standard old " not enough time in 
formalin" is the issue.  I have even wondered about variables such as we use 
recycled formalin, recycled Clearite III.

Any suggestions?

I just want to remind you..

After all of that procedure of removing the paraffin sections from the
positive charged slides.
The slides will not be positive charged anymore and not suitable for IHC.
Also the cost of the detergents and the chemicals which you used for
removing the tissue section is more costly than the slides.

This is what I would do...

Soak the coverslip off in xylene
Either  - rehydrate back to water and just wipe the tissue off - then dip
the slides in absolute and let dry..

OR   - air dry out of xylene and wipe off with a wet gauze  - then do the
alcohol dip

Whichever worked best. Just my 2 cents..

You can't use full concentrate alcohol on such specimen, otherwise you will
destroy the cells membrane.
For me I am using 80% Ethanol.

I am not ok with staining of Melanosome (HMB45) Ventana using BenchMark GX 

The expiry due date still valid and I follow the staining protocol from the
pamphlet but no good stain show even on the melanoma control sections.

Recheck the paraffin bath temperature and the paraffin melting point then make 
sure to adjust the bath temperature at 2 degrees above the melting point.

I'm looking for input on the latest theories of duration of fixation for
routine IHC.  In the past, it was thought that tissues should be removed
from 10% NBF after a max of 72 hours,  and if not immediately processed,
transferred to 70% ethanol until processed to retain antigenicity.  What is
the current thinking/trends?  Over the last couple of years, I have heard
that leaving tissues in 70% ethanol may be doing more harm than good, and
it's NOT a good idea to leave them in alcohol for extended periods of time.
I would appreciate your input, or point me towards any new data.

Apparently while I was out on disability a CAP ER/PR PM2-A 2014 did not get
done.  Somehow one of our pathologists got it and forgot she had it.  That

According to my understanding and practicing, the PT must cover all your test 
menu: Surgical Pathology, Cytopathology  Immunohistochemistry.

ANP.02000   Education Participation Phase I
As applicable, the laboratory participates in a peer educational program in 
anatomic pathology (e.g. CAP Educational Anatomic Pathology Programs).
NOTE:  The laboratory should consider participation in programs appropriate to 
its scope of service.  Such programs provide valuable educational opportunities 
for peer performance comparisons in both technical and diagnostic arenas.  
While none of these completely emulates the precise clinical setting involving 
anatomic pathology preparations and rendering of anatomic or clinical 
diagnoses, they can be a useful benchmark of peer-based performance in a 
national database.
Evidence of Compliance:
✓   Records such as CAP order form, purchase order AND 
✓   Completed/submitted results indicating that the laboratory is 
participating in a CAP educational AP program OR records of 
enrollment/participation in another AP peer educational program OR records for 
participation in a laboratory-developed program by circulating case material 
with other laboratories or within the laboratory's own practice with 
documentation of peer review


CYP.00125   PT ParticipationPhase II
For laboratories subject to US regulations that perform gynecologic 
cytopathology, the laboratory and all individuals who examine gynecologic 
preparations participate in the CAP Gynecologic Cytology PT Program (PAP PT) or 
another proficiency testing program in gynecologic cytopathology approved by 
CMS.
NOTE:  This checklist requirement applies only to US laboratories and other 
laboratories subject to CLIA regulations.  Laboratories must maintain 
documentation of PT performance for at least 2 years.  Documentation must be 
kept for each individual participating in annual PT, including identification 
of those who are retested; documentation of remedial training; records of 
imposition of limitations on slide examination; and records of re-examination 
of slides, as required by CLIA. 
Evidence of Compliance: 
✓   Written policy describing handling of PT failures (may include 
retesting, remedial training, and imposition of limitations on slide 
examination) AND
✓   Records that the laboratory is enrolled and all currently employed 
personnel have successfully completed PT AND
✓   Records of retesting, remedial training and imposition of limitations,

✓   Records of notification to the PT provider and CMS for any PAP testing 
personnel who left employment prior to completion of annual PT

On a recent CAP inspection, we did have an inspector ask to see our slides
from our HER2 validation.  For the past 5 years, we have kept all of the
slides from our validation studies along with the paperwork in a notebook.

Interestingly enough, on a recent Joint Commission inspection last year, an

give safety orientation for every new staff which include instruction about 
handling the food and drinks in the working areas and the disciplinary action

Recently we have been experiencing a problem when running HE slides on our
Gemini stainer (Thermo Shandon Varistain). At the end of the run, small
water droplets or possibly air bubbles are seen on the slides after taking
the slides out of xylene. If they are coverslipped, a white haze will be
seen around the specimen after a few minutes. Any suggestions of what
causes this and how to correct it? Thank you!

Dear Histonet,

I am having a major problem with bubbles in my mounting media. I am
currently using the Leica coverslipper and Cellpath mounting media (Phalate
free). Micro-bubbles are observed on the slides after sometime of
coverslipping. In fact, when I checked the media, I found lots of bubbles
inside the bottle too! There were no bubbles in the bottle prior to placing
the mounting media with the machine (I checked!). Also, we do not see any
bubbles in the tubes that lead to the probe!

Previously, we were using the Leica mounting media (yellowish liquid) and
there were no bubble issues. I have checked with other labs using the
Cellpath mounting media (they use Leica coverslipper too) and no one seems
to be having the same problem as us!


The Cellpath mounting media is way more viscous compared to that of the
Leica media.

Has anyone here experienced a similar issue, and how have you resolved it?

why we have a policy with simple procedure and record, the procedure almost

 I've never been asked by a CAP auditor either, but our internal inspectors
used to inquire about this in a previous lab all the time.  We use
Bromocresol Purple as an indicator here, and we have a simple log sheet
wherein our crew just tests one piece of cleaned glassware each day.  They
write the type of glassware they test, and place a checkmark in a yellow
or purple column based upon what they see.  If purple, they rewash,
retest and document accordingly.

Liz had some great suggestions! The one that I have found to be the MOST
important is #4.  Standing my slides up at a slight angle and allowing the
moisture to drain out from under the section AND then letting the drained
slides sit and dry for a good long time is extremely important for adherance
to be successful (I've never gone as far as a week, although it couldn't
hurt, but at least overnight seems necessary).  I would focus on that, so
you first get good adherance and then the other tips following the proper
drying will then help keep them on.  Good luck!

We are staining human FFPE brain sections for Her 2 Neu and we're having a
problem with tissue coming off the slide during the HIER step.
We are currently using Vectabond coated plus slides and using a decloaking
chamber for HIER.
Are there any other steps we can take to prevent  tissue coming off the
slide?

There are a few things that you can try.

1.  This may or may not work with the Her2 IHC, since for that antibody I
believe it requires retrieval at least at 98C but time and temperature are
related with respects to heat retrieval.  You could try 70C for 2 hours this
is gentler on the tissue and works for some antibodies but not all of them.
For example it does not work for Ki-67 in our hands.  We use this retrieval
method on bone and other types of friable tissue if necessary.  

2.  We have found that some samples after HIER may require manual staining
methods in order to optimize tissue adherence, not sure if that is an option
for you too.

3.  Post fixation in 10% NBF after deparaffinization for about 10% minutes.
This may help tissue adherence but we have also found that this may decrease
the sensitivity of the protocol and you may have to adjust your primary
antibody concentration.

4.  Tissue adherence to the slide especially with brain (we have seen this
on rodent brains) requires proper fixation and adequate processing, poorly
fixed or processed brain samples will have issues with tissue adherence.

5.  Allow the cut slides to sit for about a week prior to staining we find
this may help with tissue adherence.

[Histonet] Validation of IHC

Dear colleagues

We purchased Ventana GX from Rosh for IHC

I reviewed the resent CAP check list 2013 for validation of IHC but I got
confused.

I am using ready to use prediluted antibodies from Ventana and I want to
validate the antibodies before use according to CAP requirements.

Can I have the validation plane and procedure for IHC antibodies.
