Hi If you are using buffered Formaline so the Eosin color will not resist until the end of tissue processing.I recommend to add small amount of stock Eosin to the last alcohol in theTissue processor. Good day colleagues I need your help in collecting the data for the bellow requirements for CAP, is their good reference to collect the benchmark statistics for ER, PR & Her2neu ? ANP.22970 Annual Result Comparison For immunohistochemical and FISH/ISH tests that provide independent predictive information, the laboratory at least annually compares its patient results with published benchmarks, and evaluates interobserver variability among the pathologists in the laboratory. NOTE: Individuals interpreting the assay must also have their concordance compared with each other and this concordance should also be at least 95%. With specific reference to estrogen and progesterone receptor studies: in general, the overall proportion of ER-negative breast cancers (invasive and DCIS) should not exceed 30%. The proportion is somewhat lower in postmenopausal than premenopausal women (approximately 20% vs. 35%). The proportion is considerably lower in well-differentiated carcinomas (<10%) and certain special types of invasive carcinomas (<10% in lobular, tubular, and mucinous types). The proportion of PgR-negative cases is 10-15% higher than for ER-negative in each of these settings. Investigation is warranted if the proportion of negative cases is significantly lower in any of these settings. Jamal M. Could it be something that is happening with the tissue before it gets to the lab? Usually a delay if fixation causes other artifacts but not bubbling. Could it be heat from the GI procedure? 2. We do use blue sponges for our biopsies. I know some say get rid of the sponges but has anyone seen this problem caused by usage of sponges? 3. What about the heat stage in our Prisma stainer? I am really getting frustrated. Pathologists never complain however I would rather all of the tissue did not have the "nuclear bubbling". Again we only do biopsies so I really don't think the standard old " not enough time in formalin" is the issue. I have even wondered about variables such as we use recycled formalin, recycled Clearite III. Any suggestions? The slides will not be positive charged anymore and not suitable for IHC. ANP.02000 Education Participation Phase I As applicable, the laboratory participates in a peer educational program in anatomic pathology (e.g. CAP Educational Anatomic Pathology Programs). NOTE: The laboratory should consider participation in programs appropriate to its scope of service. Such programs provide valuable educational opportunities for peer performance comparisons in both technical and diagnostic arenas. While none of these completely emulates the precise clinical setting involving anatomic pathology preparations and rendering of anatomic or clinical diagnoses, they can be a useful benchmark of peer-based performance in a national database. Evidence of Compliance: ✓ Records such as CAP order form, purchase order AND ✓ Completed/submitted results indicating that the laboratory is participating in a CAP educational AP program OR records of enrollment/participation in another AP peer educational program OR records for participation in a laboratory-developed program by circulating case material with other laboratories or within the laboratory's own practice with documentation of peer review CYP.00125 PT ParticipationPhase II For laboratories subject to US regulations that perform gynecologic cytopathology, the laboratory and all individuals who examine gynecologic preparations participate in the CAP Gynecologic Cytology PT Program (PAP PT) or another proficiency testing program in gynecologic cytopathology approved by CMS. Also, we do not see any bubbles in the tubes that lead to the probe! Previously, we were using the Leica mounting media (yellowish liquid) and there were no bubble issues. I have checked with other labs using the Cellpath mounting media (they use Leica coverslipper too) and no one seems to be having the same problem as us! The Cellpath mounting media is way more viscous compared to that of the Leica media. Has anyone here experienced a similar issue, and how have you resolved it? Appreciate all the help I can get! Standing my slides up at a slight angle and allowing the moisture to drain out from under the section AND then letting the drained slides sit and dry for a good long time is extremely important for adherance to be successful (I've never gone as far as a week, although it couldn't hurt, but at least overnight seems necessary). This may or may not work with the Her2 IHC, since for that antibody I believe it requires retrieval at least at 98C but time and temperature are related with respects to heat retrieval. You could try 70C for 2 hours this is gentler on the tissue and works for some antibodies but not all of them. For example it does not work for Ki-67 in our hands. We use this retrieval method on bone and other types of friable tissue if necessary. 2. We have found that some samples after HIER may require manual staining methods in order to optimize tissue adherence, not sure if that is an option for you too. 3. Post fixation in 10% NBF after deparaffinization for about 10% minutes. This may help tissue adherence but we have also found that this may decrease the sensitivity of the protocol and you may have to adjust your primary antibody concentration. 4. Tissue adherence to the slide especially with brain (we have seen this on rodent brains) requires proper fixation and adequate processing, poorly fixed or processed brain samples will have issues with tissue adherence. 5. Allow the cut slides to sit for about a week prior to staining we find this may help with tissue adherence. Good Luck Liz Elizabeth A. [Histonet] Validation of IHC
Dear colleagues We purchased Ventana GX from Rosh for IHC I reviewed the resent CAP check list 2013 for validation of IHC but I got confused. I am using ready to use prediluted antibodies from Ventana and I want to validate the antibodies before use according to CAP requirements.
