Considering the changes between 1.4.0 and 1.4.1, this seems unlikely.

Could you give it another shot and see if you see the same behaviour?

We run a Saxton Hale server and a lot of client crashes and some
server crashes point in the direction of custom player models. In
particular it seems to happen when a player with a custom model dies
on the client, while I'm not quite sure on the server. I'm also rather
scared of the bone setup code they have since it seems to crash one of
our gamemode servers constantly and could be related to the client
crashes as well.

 If it turns out to be an issue with player models (Not impossible
 considering how there are new attachment zones made for them all constantly
 with the new items that we've seen recently.) then narrowing down this will
 hopefully allow valve to patch it correctly.

No, the random drops of clients isn't related to this. If there is such 
exploit, it's a separate issue. Not everyone crashes off on our servers 
when the crash event occurs. Sometimes they all do  crash though.

Yep - been seeing this more lately. 12-15 players all crash at once, with
no discernible cause. At this point, I think it's an engine/steam issue,
but who knows - there could very well be another exploit in the wild.
Hopefully, the TF2 team is aware and tracking down a cause.

For what it's worth, if this helps... I have noticed that my Linux servers
have this crashing issue whereas my Windows server does not. Also, I just
recently got around to updating SM from 1.4.0 to 1.4.1. Once I did this,
the crashes increased by 10 fold and I was forced to downgrade back to
1.4.0.

All of my servers are windows 2008 R2 64bit, and the problem persists on
them, so I don't think it's an OS issue.

crashing out.
