Can confirm. You can actually create a top level folder though Akonadi Console.
Also if you rename the account in Kmail it will work for a short period of time
before breaking again.

Yep. Disabled adblock in Konqueror. Must have enabled it at some point but I
never use Konqueror so I wouldn't have guess that. I had to also remove any
blocklists laying around in ~/.kde/share/apps/kmail2

Not sure, it didn't get to the drop down, and in fact just said 'new...'
When I open it now it's looking very different. When I press down to go
to 'Todo lists' it crashes. 'To-do List' doesn't crash
in a separate window - this hasn't crashed.

Yep I confirm that for me (see comment #3) the lack of icons is on an Exchange
DIMAP server. Having read #4 I also setup a local DIMAP server on my Linux
mailbox as a test, and that *does* get
