Ok, I will upload a simple example (example.zip). Here are the steps to show
the bug:

1) Open "example.odm
2) Click "Yes" to update links
3) Go to the end of the master document
4) Right-click on "blank.odt" in the Navigator pane and select "Insert index"
5) Choose "Alphabetical Index" from the Type menu
6) Click the "Concordance file" check box
7) Choose "Open" from the File menu
8) Select "words.sdi"
9) Click the "Columns" tab
10) Click the two-column icon
11) Click "Ok" to generate the index (it will have only one column)
12) Right-click within the index and choose "Edit index"
13) Click the two-column icon
14) Click "Ok" to re-generate the index (it will have two columns)

Wasn't clear, that your report is about master document and subdocuments. Could
you plase provide a short sample document, so we're able to reproduce the
problem.

No, it does not solve my problem. I have a master document containing linked
subdocuments, and I have a concordance file of almost 4000 significant words
that I want to include in an index at the end of the book. So I go to the end
of the book and insert an alphabetical index, specifying 2 columns with a small
gap between them, and select the concordance file. After several minutes pass
(the book is over 300 pages) the index appears with only one column. Then I
have to edit the index and again specify 2 columns, and regenerate the index.
Then several more minutes pass and the index appears with 2 columns. 

Having to generate such indexes twice for each master document is a bit
tedious, but I can live with it. LibreOffice is an amazing application and I am
very thankful that it works as well as it does. It's my principle tool as a
writer/editor, which I've been using for the past 15 years since I abandoned
the Microsoft world. I appreciate the labors of all who continue to develop it.

Thank you for the file, Bogdan. It works for me now. I've also tested following
steps:

1. Open an empty document
2. insert alphabetical index with two columns
3. Copy and paste text from attachment 178361
4. Update index

Result: Index with two columns

=> David, does these informations solve your problem? If not, please give
precise steps to reproduce.

The first video was not ok.
I upload a new video.
You can see I am playing in the video with 2 or 3 columns and everything is ok

Just tried it in version 7.1.1.1. It let me choose 2 columns and specify a 0.1
inch gap between them, and then it created the index with only one column.
(This is an Alphabetical index, by the way, using a concordance file.)
So then I edited the resulting single-column index and again specified 2
columns with a 0.1 inch gap, and this time it generated the index with two
columns.
