Formatting name/date to be filled by others

Before I re-invent the wheel I want to discover if there are existing tools to provide for entry of name, signature, and date to be filled in on a printed copy of the document page. Specifically, this is for project proposal acceptance and should look something like this (in fixed-space font):

Buffer-export works in 2.3.0!

A sequence of multiple buffer exports works in 2.3.0 -- they did not do so in 2.2, and I had to write a shell script to circumvent it (that sometimes worked, sometimes not). For example now I have the keyboard shortcut command-sequence buffer-write; buffer-export pdflatex; buffer-export pdf2
