\maketitle (or [EMAIL PROTECTED], as I've seem sometimes -- don't know the
difference) macro. It's quite easy if you know some LaTeX (from head,
might contain typos):

Did you ever get a reply to this? If not, it would be good to export,
re-import, save as a new LyX file, then compare the two LyX files to
isolate what the difference is.
For example, if I go to 'Insert-Citation reference' and select a
citation it
