Spent a few minutes looking at vacuum connectors (as suggested) and found a vac hose disconnected from a small green canister thingy located over the GP Relay - the tubing had been pulled loose at some point and was difficult to find since it is located under some other hoses and wiring.

Actually, the accumulator spring in K1 will fix your 2-3 "stacked" shift, and replacing the accumulator spring in K2 will soften and slightly delay your 3-4 shift. You can do one or both, it's not mandatory to do both. For sure do K1, as it will make a noticeable difference in the 2-3 upshift.
