In my experience some users have problems with command line tools or even never
opened the cmd shell.
By the way, the "qgis.db" in the "privat" profile directory can deleted without
issues, if it is broken. QGIS will create a new one on startup.

I've never seen this error before, but I suspect your DB file may be broken.
I/O errors could also indicate a corrupt file system (on your hard drive), so
you should check that first (right-click your drive in Windows Explorer and
select Properties, then Tools).
