It may well be, as I rarely cut the cord before the
placenta has birthed during a physiological third stage (unless requested), even
when the woman chooses not to have a lotus birth.

Hey Brenda
Why on earth would you sit down for that.
Where are your consumers running to the media.
It is only at election time will we achieve anything!

I've tried phoning Ryde but no one's
answering either today or yesterday. Can anyone answer the question yet? Is it
closing?

Hi,
I'm interested if there is any research on delaying synto for say up to
5 minutes in 'active 3rd stage'.
Have been doing actively managed third stage throughout my clinical
placements as a student (nearly finished!!) with some practitioners
cutting the cord immediately, and most at about
Hello Sue,
The question of third stage management has a cultural aspect as well.
In the US, as far as I know, the syntometrine is not given until
after the placenta arrives. It is then given to prevent excessive
bleeding. Interesting to speculate on how this major difference
developed, and
Sue,
You really need to decide whether your patient wishes for an active or a
physiological third stage. It can be dangerous to try and do a half-half
sort of third stage.
If you plan a physiological third stage then you need to:
1. Dont clamp or cut the cord until it stops pulsating.
2. Put
Sue that's how I do most of mine...don't give the oxytocic with the anterior
shoulderand it's usually some minutes before I 'get' around to it!!!
There is a wide variation of practice isn't there...it is one of those
things that we have always been led to believe i.e if you don't give that
Hi Sue this is something I have always been interested inthe link is to
the FIGo/ICM Joint statement on active third stage and it states you can
leave the cord in a well newborn to stop pulsating...and then cut; this is
with giving the oxytocic at birth..
Title: Re: [ozmidwifery] No Births at Ryde Birth Centre
Dear All
Ryde Midwifery Group Practice is not closing. For the immediate future all women booked on the RMGP will need to birth at RNS.
I believe Ryde Hospital is having some issues and there is a view to remove the RMGP from greater
I should add that a physiological third stage is the natural progression
from a normal labour.
However if the woman has had a labour influenced by IOL, augmentation and
directed pushing in second stage then she is more at risk of a PPH because
her uterus has been unnaturally pushed to labour
Competely agree Leanne...but am also of the belief that the issue of timing
of the injection, and therefore timing of the clamping are things that have
been thrust upon us. The research is variable in its support of when to give
the oxytocic and how long we can leave the cord to pulsate after
Why would you do that Lisa? if you give an oxytocic you get a surge of
blood through the cord. The uterus contracts within 3 mins Syntocinon 7/9
mins ergometrine. The Placenta should be sheared off the wall within
minutes. If you want the cord to stop pulsating in a well new born surely
Where I work when I am not being a MIPP we leave the cord until it
has stopped pulsating, or the mother asks for it to be cut and then
we give the synt after it has been clamped. Our PPH rate is not
noticeably higher
Put it this way we have the usual conservative GPs and they do not
object
Well that's what we've always been led to believe, but it's not a one way
system is it? So if you are using syntocinon (not syntometrine where the
contraction is much stronger and more sustained) why won't the baby regulate
it's own blood volume? I am speaking from hospital experience where
Hi Lisa, Delaying the Syntocinon until after the cord has stopped pulsing
isn't a problem as up until you give the drug you are treating it as a
physiological third stage. giving the drug then just waiting for the cord
to stop could be a problem. When I have a few mins I'll check it out I'm
I have a PDF of the full paper if anyone is interested..pm me

Hi Lisa sorry didn't mean to reply as I did, am abit passionate about
this..have pmed you a copy of the bmj paper which talks about this..I think
what they are saying is that the 'belief' about the overload of blood with
the oxytocic hasn't been proven maybe!? Have read lots of old stuff about
the
I know! But some get the poops and can't be bothered waiting..

I realise that some women opt for active third stage but if they've gone as
far as letting the cord stop with no bad effect it seems a shame not to
continue.
