the beyond operator yields true only if the considered atom pair
distance is beyond the threshold in ALL states. To remove atoms which
are beyond threshold in ANY state, try this:

I don't think that PyMOL has an automatic renaming mode for those names.
But you can rename them manually:
alter name O1P, name=OP1
alter name O2P, name=OP2
