I figured out what was happening, I had to allow spamassassign traffic
thru.

-A INPUT -p tcp -m tcp --dport 53 -j ACCEPT, Change your line with this: -A
INPUT -p tcp -m udp --dport 53 -j ACCEPT
Becouse, DNS queries is using the UDP protocol and port 53.

I see that this port is in the standard toaster firewall, but that it's
commented out by default.

contributed to the qmailtoaster.
I did a search of the system, and I don't have any .qmail files in
any of the problem accounts. I should also note, that I am not
running TLS, Spamd, clamd, or the likes on this box.

Smells like dns to me too. But not usually for pop3. You say this is on
a local lan. Can you resolve to the servername quickly? Maybe put a test
