Yup, you are correct.
The file is there and it has only a single line of text.
This user has exceeded their mail quota.
However when it arrives in the users mailbox it has not from
address and no subject and even worse no body text.
Any thoughts?

as you deem fit).

That looks very malformed all right.
Are you certain it came from your QMT?
There's not even really enough in the headers to tell where it
originated, or how it entered the system. Can you backtrack it in the
logs? Starting with the send log, then the smtp log.
What are your toaster versions,
