Then .qmail file is not the cause of the duplicate delivery.

I thought some issue on qmail-send and send folder on the log folder as
well /var/qmail/supervise folder. Why it's shows two folder.

That's indeed peculiar. Like I said, I've no idea how your system came to
 have qmail-send and smtp2 directories under /var/qmail/supervise. I would
move them somewhere else safe (as opposed to simply removing them) and see
 what happens:
 # service qmail stop
 # mv /var/qmail/supervise/qmail-send /root/.
 # mv /var/qmail/supervise/smtp2 /root/.
 # service qmail start

is any other way to fix or find this issue.

See if moving those folders remedies the problem. If not, I would suggest
finding corresponding entries in the smtp logs to verify whether or not the
duplicates are coming into the server twice or not. Use qmlog to search the
 logs (-lc option).

Are these duplicates intermittent, or always?
 All or some accounts?

What is the load on the server?

Have you mentioned which versions you're running?
 # rpm -qa | grep toaster

The second cause might be excessive clamav scanning times. If clamav
takes too long to scan, the sending client (be it an external server
or a submission client) can time out the connection thinking the
message hasn't been sent, and retry. Upon eventual successful
completion of scanning, QMT will deliver the message, but fail to
notify the sender of such because the sender timed out. After a period
of time, the sender will re-try sending, resulting in a duplicate. The
most common cause of this I've seen is with Outlook. Outlook clients
should be configured for maximum time to time out, which generally
fixes the problem (without really impacting Outlook performance).

Question : when you say excessive clamav scanning times would this
always be the scanning of that email

or is it possible for clamav to be bogged down with someone elses email.
meaning

email 'a' is sent , email 'a' has excessive clamav scanning time so
email 'a' is sent again .

or
email 'a' is sent but clamav is bogged down scanning a big
email/attchment of email 'z' currently making email 'a' ---wait.

timeout is exceeded and email a is sent again.

thanks

There are two situations I'm aware of that can cause duplicate deliveries.

First is the .qmail file(s). This could be the .qmail file for an
individual account (which is found in their account folder), or the
.qmail-default file for the domain (in the domain folder). If this is the
cause, the solution is to edit the errant file manually, removing the
 inappropriate delivery line.

The second cause might be excessive clamav scanning times. If clamav
takes too long to scan, the sending client (be it an external server or a
submission client) can time out the connection thinking the message hasn't
been sent, and retry. Upon eventual successful completion of scanning, QMT
will deliver the message, but fail to notify the sender of such because the
sender timed out. After a period of time, the sender will re-try sending,
resulting in a duplicate. The most common cause of this I've seen is with
Outlook. Outlook clients should be configured for maximum time to time out,
which generally fixes the problem (without really impacting Outlook
 performance).
