It sounds like you want to rotate the lever mounts 90 degrees on a mustache bar. I’ve never seen this done, but it seems feasible. You might want to adjust the mating surface of the lever with a half-round file. Disclaimer: I didn’t even visit the bike shed to look at the pieces. UPDATE

My wife's pbh is long for her torso (relative to many folks), and she's prone to neck and shoulder pain if too stretched out on a bike for more than a few miles. The other week I finally swapped out the 50mm stem on her Roadini with a 0 offset stem I bought from James/Analog a looong time go.
