Adding the index did indeed fix the issue. I've pasted the line from
the SQL log below. It looks like the query is coming from
RT::Transaction. I'm not that great at perl so I'm not sure, but I
think it might be the method LoadCustomFieldByIdentifier. I do know
that the URL that starts the query

Your logs are chock full of SQL statement execution errors, which
DBIx::SearchBuilder 1.65/usr/local/share/perl5/DBIx/SearchBuilder.pm
make testdeps indicated no problems, nor did make upgrade-database.

Is it possible to include other files which contain config options in
RT_SiteConfig.pm? Was hoping something like this is possible, like how
you'd use Include in apache configs, to make it more readable.

I cannot seem to get RT4.2 to run with FastCGI I am install the on the same
server that I am running RT 4.0.13 so I know fastcgi is enabled for apache.
RT4.2 run with fine with the following command
/opt/rt4/sbin/rt-server -port 8080
