URL, which might make it clearer where the error is coming from.
If that doesn't help, my usual checklist is to try switching to Vector (if
you're using a custom skin), and temporarily disable all extensions except
for Sem

When the "Add another" button works, it's usually a sign that there's some
other, unrelated Javascript error that happened. Can you look in the
Javascript console, if you know how to do that, and see if there are any
error messages there?
