Thanks for looking into this, I have tried to force standard mode with
!DOCTYPE HTML PUBLIC -//W3C//DTD HTML 4.01//EN
but IE still appears to fail so I used absolute urls and the icons
load on the map.

Aha! It's Internet Exploder 8 that's not displaying the icons on the
map, Firefox is displaying the icons correctly.
David, you mentioned that you see icons on the map but not the legend?
I have been able to see the Legend without issue.

On Feb
I had no problem seeing the icons (on firefox). It's just that since
the default icon shape is circular and the pins don't match your icons'
pins, you get some odd little stuff in the margins that I think will go
away if you set the shape to square.
I see the IE8 problem you mention also
