Ehm... NO kernel module:
modem_run -m -f /etc/mgmt.o (may add -v something for debugging?)
Options.. huh? added -e 1 in the pty section (pppoa3 blah blah) of
/etc/ppp/peers/adsl or whatever
No source code editing...
The drivers are the usual ones, and the microcode is mgmt.o :)

1) Yep. killall pppd (or perhaps killall -9 pppd)
2) I've got an initscript by default... it depends on the distro you
run. Check for the availability, or edit a file with these lines:
modem_run -m -f /etc/mgmt.o # Where is mgmt.o
pppd call adsl #
Recompile the kernel omitting that module... fyi it is better to
have an updated version (2.4.25 for example), cause some kernels
have embarrassing vulnerabilities (such as 2.4.24 ones).
