I had the same problem on RHEL4 and on Centos 4
My solution was to just grab the latest srpm from Fedora Core 4 Updates,
did a rpmbuild and upgraded the so STABLE10 10 minutes of work and no
problem afterwards...
I really wonder why there is was update from RHEL

I think that is not all of your auth and acl lines in your squid.conf.
Your problem looks like you got ntlm auth and basic auth configured and

i'm noticing that there is a heck of a lot of activity by the
authenticating perl program when i run 'top'.
